I believe this to be the culprit, yes (and thus - a bug in Elasticsearch). The question of subfolders you may or may not have under each is simply a question of how elasticsearch decided to split the data between the folders.
Is the only difference you are seeing between docs is this one field existing/missing? I'd try and characterize this and perhaps figure out a way to create minimal repro the ES team can run.
