Do you have the contacts fully inserted in the housing? When pushed in the should "click" in place and cannot be removed by tugging on the wire. Try pulling the wires out of the shell, if the wire and contact comes out it wasn't in all the way. If they aren't fully seated, you can get all sorts of intermittent connections!
