Page 74 of the manual covers the installation of the option headers quite well. This is after the PA transistors have been mounted and before the installation of the heatsink. Do it just as the manual indicates and there will be no problems.
As an additional hint to make things easier - on page 75, you will remove the nuts from the PA mounting screws prior to adding the thermal pads and installing the heatsink. There is a caution about not letting the PA mounting screws fall out during this process. To aid that, cut (do not tear) a piece of electrical tape about 1 inch long - stick it to the screwheads and the RF board (and T4).
