Silly question perhaps, but did you make sure the fan cable is plugged in? See the figure "Connecting the Fan Cable to the Power Supply Module." - fig 37 on page 27 in the Rev D manual.
An ohmmeter at the fan connector will tell you if there is continuity through the fans. If the fan runs when you shut the KPA500 off at the front panel, then you know the wiring is good.
