I'm in the computer business and have been for 30 years. I have never seen such a device. However, using something like Arduino Leonardo <http://arduino.cc/en/Main/ArduinoBoardLeonardo> one could easily create such a thing using a common 3x4 keypad and a fairly small amount of code. There are probably other ways to remap a keyboard. There are numeric-only keyboards, but I don't know of any simple way to remap the keys to something else.
