If you want really good in-ear earphones, look at offerings by Etymotic Research and Shure. I'm happy with all but the lowest cost Shures. The Shure products are widely available in the retail distribution world. I think that Etymotic also has dealers, and the dealers may offer substantial discounts. All of these pro products work fine with the ear-fittings that come in the box, but can be fitted with earmolds. You need to get fitted by an audiologist. Whether an earmold helps, and how much it helps, depends on everyone's individual ear. It's sort of like shoes. :) One of the major advantages of these in-ear products is superior isolation from room sound. 40dB is typical of the best, and custom earmolds tend improve on that.
