Minnesota Public Radio has this excellent piece on their website
showing 15 (so far, they may be adding more) controversial ballots.

All are based on some form of optical scan paper ballot in which the
voter is supposed to fill in a bubble.
Some of these people must not have tested well in school.
But seriously, these are some of the things that can go wrong with an
otherwise pretty simple paper ballot.
I hope some good thought goes into future ballot design based on
things like this.
