Enabling many dup-btrees to be created/destroyed is an important
performance issue for all the new features of elephant. I know Alex
mentioned this should be possible but he hadn't done it yet. I'm not
sure if CL-SQL has sufficient support yet (I think so based on what I
remember of the design - this should fall out naturally).
