Addressing only this example, and not assuming that doing so
resolves both issues as reported, I think the way to use
version 5 is to approach the matter slightly differently.
So instead, type one of:

1.
"procedure A" C-c C-e

which ask whether you intend to type a body or a spec.
Indicate body and you'll get

procedure A
is
begin
^end A;

^ indicating the position of the cursor. This is sufficient
to satisfy the parser.

2.
"procedure A;" C-c C-b

(with semicolon) which produces, without further ado,

procedure A
is begin
   null;
end A;

Not perfect, and two slightly different fragments, I suppose,
but either result gets me going.
