OK, here a few things (this is all with a "virgin" install of both, with no .emacs file): Emacs.app has an actual Preferences panel, whereas Carbon Emacs just dumped me into a customization buffer. Granted, the preferences in Emacs.app are skeletal, but at least they're there. My wheel mouse didn't work under Carbon Emacs, but it did under Emacs.app. I'm not sure why that is. That dreaded "process-connection-type" error still plagued me in Carbon, and I need M-x shell. Emacs.app "just worked." Fortunately it's an easy fix for Carbon. Meta is mapped to Option in Emacs.app by default, so all the Mac shortcuts still work. And since I invariably use my right hand for Meta I actually like this better now, as it's a shorter trip for my pinky to hit Option than Command. That Emacs.app writes out a .plist file is kind of cool. Not exactly from a usability standpoint, but it makes emacs "feel" more Mac OS X- ish. I can tweak settings with the Property List Editor if I want to. One thing I noticed about Carbon that I liked better than Emacs.app was that C-x C-f is case-insensitive, so it would change case if you've missed the shift key. Anything to keep the completion buffer from popping up get an "A" in my book. Overall, I like Emacs.app much more than Carbon Emacs.
