Not too sure about this, cannot replicate here.  Can you get it when
invoking /Aplications/Emacs.app/Contents/MacOS/Emacs -Q ?  Do you need
to be in a special mode?  What are your keyboard layout and system
input method set to?

Anyway, note that Emacs.app like Carbon emacsen supports OS X-based
input in addition to standard emacs input methods.  But you must set
Alt/Opt = "none" in preferences so that key is passed to the system.
Then, e.g. in US standard keyboard layout alt-u o makes an ö.
