Playing with "Arrange Visible Frames" and "Arrange All Frames" I  
tripped over this bug and as I was looking for a reproducible  
sequence I stumbled upon this one instead.  It fails about as often  
and in the same way but is a little easier to describe:

        1) Launch Emacs.app
        2) Choose "Help" > "External Packages"
        3) Choose "File" > "New Frame" (use the menus; it seems to happen  
less when you use Apple-N to make the new frame)
        4) Try to click the "Windows" menu bar item (I've been clicking and  
holding down mouse-1, if that's relevant)
        Expected: "Windows" menu drops down
        Observed: menu bar item highlights but menu does not drop; moving to  
other menus (while still holding mouse down) does not work either.   
Release the mouse and click again and it will eventually work though  
it may take more than one (more) click.

The same thing's happening with both of the "Arrange" items after the  
arrangement is complete.  You might have to actually move one of the  
newly arranged frames to run into it there (though I saw it once  
without moving anything).
