The node Change Logs in the Emacs manual uses "entry" and "item"
inconsistently. Maybe someone with a good idea of what things
_should_ be could correct that node?
Here are a few examples to illustrate the problem:
First an "entry" is defined a something starting with a
date-name-email header line and an "item" is defined as something
starting with a whitespace-star. Then this example is shown:

* man.el: Rename symbols `man-*' to `Man-*'.
(manual-entry): Make prompt string clearer.
* simple.el (blink-matching-paren-distance):
Change default to 12,000.

* vc.el (minor-mode-map-alist): Don't use it if it's void.
(vc-cancel-version): Doc fix.
It is supposed to comprise "two entries, both dated in May 1993, each
with two items". I count one item in the second entry. A bit further
on, it is said that "the second entry above contains two items grouped
in this way", which also suggests a different definition of "item".
It is also said that "When the variable `add-log-keep-changes-together'
is non-`nil', `C-x 4 a' adds to any existing entry for the file rather
than starting a new entry." Here, "entry" should probably be "item".
