I tested piem again and I now better understand what it does. I discovered b4 too, and the way it builds git-am-ready patches from public-inbox - very nice. Part of b4 job is done by BONE when collecting patches/series, so not all b4 features are useful in gnaw.el. Still, gnaw.el now can apply patches on branches and worktrees.
