I agree with you about not making nontrivial changes before the
    release.  But I don't think that it is not worth the trouble in the
    long term.  First, images are usually displayed using the `display'
    property, and overlay strings are not shown before them.

It seems to me that the right thing is that a before-string on the
first character under the image and an after-string on the last
character under the image ought to appear.

Does anyone disagree?
