Here is a copy of an email that Apple sent out at the time DFA 8.6 came 
out.  IIRC, the problem starts showing up when the startup drive has more 
than 100,000 files on it and is fragmented.

As part of an ongoing commitment to deliver robust desktop client 
management solutions, we are releasing an updated version of the Disk 
First Aid utility to correct problems experienced by some customers using 
AppleShare IP and Macintosh Manager. 

Symptoms: 

 On hard disks initialized in Mac OS Extended format while running Mac OS 
9, users may experience a hang very early in the Mac OS startup 
process.Typically this occurs when a hard disk is nearly full, if there 
are a very large number of files on the drive, or there is constant disk 
activity such as with a busy AppleShare IP server running Macintosh 
Manager. 

Solution: 

 If you are using Mac OS 9 and have experienced difficulties booting your 
system from a hard disk formatted in Mac OS Extended format, we encourage 
you to download, install and run Disk First Aid 8.6. This new version of 
Disk First Aid is available via manual download.
