I have had similar problems and have found a cause: You should use DVD-R disks which are rated NO FASTER than the drive speed.
I have had endless problems trying to write 16x disks with my 8x drive. The 16x disks work fine in a 16x drive and my 8x drive works well once I bought some 8x disks.
