In your page you say

[! Execute ({isa => 'title.epl'}) !]

In your title.epl you say

[! Execute ({isa => '../title.epl'}) !]

That should do the the trick of setting up @ISA

BTW. I would use 

<title>[+ $req->title() +]</title>

And then return the title string instead of print OUT
