Yes, usually it is because it was not shut down nicely. Electrical storm,
power outage or something like that. The controllers are on surge
suppressors, but not on a UPS.

Interesting. Over the last couple months I rebuilt the two machines on which
I saw this the most, and brought them up to 2.3. It is true that I have not
seen this since they were rebuilt. But given how rarely it occurred, that is
not unusual.

If it is not a problem under 2.3, then so much the better. I will keep an
eye on them and see if any 2.3 machines experience this problem.
