Well, just my 1 cent worth would be that the "intent" of the standard would also include power lines crossing over telephone lines on a toppled telephone pole. Or, as we recently saw, even 16.6KV in the case of some druggie. <I think the post was extremely appropriate for this forum.> I've seen the aftermath of a transformer taking down most the wires around it on it's way to the ground. Not a pretty sight. The "TNV" circuits are certainly within the product. But, I'm sure I could give a good argument over the definition of "exposed" regarding to what they attach and what potential hazard could occur.
