Chris,
It is not a requirement to be seperate. You can still use a double
nut/stud combination, but the green/yellow must be the bottom wire, and have
its own nut and lock washer. The rational is pretty straightforward. The
main idea is that this wire not be removed during servicing, because someone
may forget to put it back on, and this is the primary mechanism to make
certain that the fault current trips the breaker and removes the power.
Check your beer mug immediately to make sure its attached. I've seen
people fall off of bar stools occasionally, and I am fairly certain improper
grounding was the cause.

I totally agree with your views, which you put much better than I.
Just to be different/arkward, I always insist that the green/yellow wires
have their own terminal. I'm sure that's not a 'requirement' and not
always practicable, but I just don't want people fiddling with the
protective wires, losing nuts, etc.
