You can obtain both of these reports (and many more) from
http://www.cbscheme.org/cbtrep.htm <http://www.cbscheme.org> . It has been
some time since we have obtained blank test reports, but as I recall, there
was a fairly reasonable fee for the reports and many of them could be
E-mailed to you.
