Jeff, I assume EN61000-4-11 is the same as IEC61000-4-11:1994.
Scope of IEC61000-4-11 says: " This section of IEC1000-4 defines the immunity test methods and range of preferred test levels for electrical and electronic equipment connected to low-voltage power supply networks for voltage dips, short interruptions and voltage variations. The standard applies to electrical and electronic equipment having a rated input current not exceeding 16A per phase. It does not apply to electrical and electronic equipment for connection to dc networks or 400Hz ac networks. Tests for these networks will be covered by future IEC standards. The object of this standards is to establish a common reference for evaluation the immunity of electrical and electronic equipment when subjected to voltage dips, short interruptions and voltage variations". The scope does not say anything about that only single-phase applies, but in chapter 8.2.1 they are talking about 3-phase equipment and "... all three phases must be simultaneously tested ...." I would say that -4-11 applies to both single- and three phase equipment.
