Hi John, 

I have a copy of IEC 1000-4-1:1992 (French/English version), and it lists
clause A.1.1.4b on page 43.  It makes reference to Figure A.2, which is on
page 47. 
It's lacking many details, including test levels.  However, this standard is
titled an 'Overview'  - it's not intended to cover details. 

Looking through the IEC catalog for immunity to low-frequency harmonics, I see
IEC 1000-4-13:2002 "Testing and measurement techniques. Harmonics and
interharmonics including mains signalling at a.c. power port, low frequency
immunity tests". 
The first few pages of the standard, including the scope, can be found at:

You might look at this standard and see if it helps.

The UPS standard EN  50091-2: 1995 has a requirement for low frequency
immunity tests per subclause  A.1.1.4b of IEC 1000-4-1. 
I have a copy of EN  61000-4-1 but it has no test methods and no subclause 
A.1.1.4b.  
Does anybody know  what spec EN 50091-2 is referring to and where to get a
copy or just what the  test method is for coupling the LF signals onto 3 phase
 equipment?
