I do not understand how y-caps can "arc", as most Y2-rated caps that I have
tested will withstand 3kV di-electric and 4kV surge/EFT.

Was the arc to the cap or to the lead or PCB trace ?

I have also note that the resultant voltage from some primary components to
gnd/chassis can much greater than the induced transient due to test config
and probe placement.
