This is not a regulatory requirement but you need CTIA approval fro CDMA
phones and PTCRB approval for GSM phones before one of the carriers will look
at the phone and accepting it in their network.

I do not know about CDMA phone, but GSM only.

Definitely you will need SAR report (FCC OET65). In
addition, you may consider IEEE 1725 for battery
safety. It has not come into regulation yet but CTIA
is considering as mandatory requirement to get GSM
type approval this year.
