As part of our regular review of our corporate Product Safety Policy, I would 
like to do a quick high level benchmarking what others are doing regarding 
product safety in their companies.

1. Do you have a company Product Safety policy, approved at Sr. Management 
lever?
2. Do you have a product safety committee? Company wide or just 
product/business group focused?
3. What organization does the product safety function operate in? (QC, Product 
Engineering, Supply Chain, etc.)
