Wrote some code for a grad student study of accident demographics last
year - what a bunch of maroons. The U.S. BLS has section for injuries &
fatalities - data is not very recent, is coarse, but is easy to parse. U.S.
OSHA has a several accident/fatality incident data listings on its various
sites. Canada workers comp boards publish data - formatting sucks and is too
coarse, and you will probably have to purchase a report. Typically, specific
details of incident will be released only as result of tort or criminal
proceedings. Otherwise a FOI may have to be filed.

Across the pond, the Irish electro-tech council has a somewhat decent
database. There are probably many other EU databases.

Perhaps a well-connected individual in the IEEE could help.

And add to the industrial injury data that I cut my finger on the milling
machine this morning.

I am looking for statistical data on workplace injuries attributed to
failure of the safety related parts of the control system. I want to use the
data as source material for a paper I am working on for the 2012 SIAS
conference in Montréal this coming October.
