What pointed me toward the problem was watching the stepper motor knobs spin the wrong way: up to half a turn before the motor goes THUNK and abruptly spins the correct way with (obviously) no decel or accel ramping.
So, in general, this is not a subtle problem where the direction signal is off by a smidge, no big deal, just one step the wrong way. Unfortunately, it's also not very frequent and capturing all that HAL trace data will take a while. Worse, the data probably won't all be from the same oops.
