Well, I tried to reproduce the problem here with no luck. 
Whether it was sitting on the home switch or not, I got totally
reliable homing to the right place on all 3 axes tonight.  I 
spent about half an hour trying to trick it into doing anything
unusual, and had no luck.

But, the scenario you discussed has been bothering me, too.
So, maybe the index pulse sensing should trigger on the index 
pulse when the "zero count on index pulse" is off, but only
trigger when the counter is zeroed, when the "zero count" mode
is active.  That is also a bit messy, but I'll look into it.
