It can be done, but might take a bit of fiddling.

If you set up your system configuration files  with a B axis (I think
that seems to be what you have) but don't connect a motor, then you
should be able to G0 B45 (say) and see the axis display change to
suit.

The key would be a GEOMETRY=XYBZ statement in the INI fie:

However, you might be better with a full 5-axis kinematics and driven
rotaries. :-)
