I tried your tool.tbl in a sim, and get the same result.

By removing the two trailing newlines from the file, saving then
reloading from the Axis "file" menu I was able to get LinuxCNC to see
the tool.
Similarly, opening the tool-table editor, checking the file, writing
the file, then reloading in Axis also works.

I am not sure what the actual problem is, it seems like the
file-parser might be less robust than one might hope.
