Ah. I am getting confused.
You don't. That circuit (and the other one I came up with with a pair
of optos) only gives you 0 to X volts. I have only ever used it to
drive a VFD.

+/- 10V is more tricky, (and offset PWM is not a great way to do it).
PWM + DIR is a safer approach as then you don't get full-scale at
power-off.
