In the interest of getting this document out the door, I've made some updates.
Instead of trying to rework the crypto binding key derivations, I've just left them in. The bulk of the document is, in fact, unchanged from the previous revision. What has changed is some new sections which explain the interoperability issues, and how they affect TEAPv1 implementations. I hope these changes are clear and non-controversial.
