On OS X, every now and again gpg-agent gets totally wedged with respect to 
smart cards.  When that happens I have to kill gpg-agent and start a new 
instance.

I'd try debugging this at the command line first.  With the card removed,

        $ killall gpg-agent

Insert the card, then:

        $ gpg2 --card-status

(gpg-agent will be started again automagically.)

If you get back a whole ton of diagnostic information, then GnuPG sees the card 
A-OK.  If you don't, we know the problem is in the GnuPG-card linkage.

If the card works correctly with GnuPG, the next step is to try the Enigmail 
card manager again.  If it works, great!  If not, then at least we know the bug 
is in Enigmail and not in the GnuPG-card linkage.  :)
