it happens even with "mouse focus" and with every combination of the two

I can confirm this, alt-tab indeed restores keyboard input, simply forgot to
mention it :)

don't remember if I've ever tried this, I'll see next time it happens, but I
doubt I've never moved the mouse elsewhere on the desktop.
