It depends, if it's an icon that you created, then you must have either
forgotten to put the Window Class or you entered it wrong. if so, then check out the documentation : http://www.get-e.org/User_Guide/English/_pages/3.3.html there is a paragraph that says : "Note that you should _not_ set the window name. The window class needs to be correct or otherwise the icon won't be displayed in the top-left corner of the window border. You can find out the window class by issuing the following command in a terminal: "xprop WM_CLASS|cut -d \, -f 2" and clicking a window. Alternatively if you have e_utils installed, you can easily find out the window class with e_util_eapp_edit, which you can open by clicking "Create Icon" in the menu that is displayed when you left-click the top-left corner of the window decoration menu. You can also access the same menu by right clicking anywhere on the top window decoration bar. E_util_eapp_edit should show the correct window class of the application it was opened from by default." if you didn't create the icon and it's one of the defaults, then there might be another problem.
