My setup is Windows 10 Pro 64-bit running Firefox 32-bit ESR v52.0.

Michael Gordon suggested that I delete any old Firefox shortcuts and only use 
the one from the Start Menu. That worked.

I compared the properties of both shortcuts and the only difference I could see 
was that the ESR shortcut had more users and groups under Security and that 
made the difference.

Not sure why you are seeing that.
My desktop,  Windows 10 Enterprise, 64-bit
Firefox 32-bit ESR 45.8.0
Alt-Click on taskbar icon gives several choices


Open New Tab
Open New Window
Firefox


Selecting Firefox opens a new Windows with my profile defaults


Are you on ESR 45.x or 52.x?
