Did you look at any AppleScript sites?
Here is what you need:
Fill Contact from Form [v1.1.0] (42k) - If you have a website with a CGI
form that mails you the results, this script can quickly create contact
cards for each user in Entourage. It can be run as a mail rule so actions

I use this, modified slightly, to get all the form info into the notes
record and other custom fields in the contact record.
There are also some stand-alone utils that will take a dropped clipping and
convert to either tab-delimited or comma-delimited text, so you can import
into a db.
