This question concerns OS X/Classic only - I've added it to the subject.
 
I've had that happen once or twice too, but not totally reproducible. It
must be some strange interaction with the OS X Finder. Entourage wasn't
designed for it, and there will be an OS X Entourage in 7 months or so, of
course, so I don't think that any effort will be expended on it. It must
actually be a quirk of OS X/Classic itself, not Entourage, and may get fixed
there.

In the meantime, I find that if I drag an attachment to the OS X desktop, I
don't usually have any problem. Then click in the desktop, Hide Others in
the Finder menu so you don't run the danger of hitting Entourage or another
classic app, and then continue dragging the saved file from desktop to the
Finder folder you want. Then click Entourage in the Dock and back it comes.
