1) Reminders come up in Entourage, not in OQV, when Entourage is open.

2) It sounds to me like an "edited" single instance of a recurring event.
There's a bad bug with these "phantom" edited events that crashed my palm
syncing., fore example.

Do this:

Make a custom view of every event with that subject name. Hopefully, you'll
notice the rogue instance in there (You might notice more than one for the
same day, or one a day before (Thursday) another. When you track it down,
select it and delete it.
