I have the same problem, and I had it when my database was pretty small.  It
happens less often now than before.  I'm not sure what caused the change.
Maybe SR1?  I think I also upgraded from Mac O/S 8.6 to 9.04.  Last winter,
Entourage would run at normal speed for a few minutes or hours after a
restart, and it would then slow down until the next one.  Lately, I've
gotten the normal speed more often than the slow one.  I run One Click,
Default Folder, URL Manager, Spell Catcher, Copy Paste, and Finder Pop.  My
hardware is a Umax S900 with a G3/350 and 377 megs of ram.  Now, what do we
have in common?
