Adding to custom dictionary

Title: Adding to custom dictionary

I asked this question a few weeks ago. Since then, because of a corruption, I had to erase my hard drive and do a clean install.  So I lost the previous setup and ask the question again:

I do spell check on an email, select the unrecognized word, yet the “add” button is grayed out.  In Word, I created and selected a custom dictionary.  In Entourage->Preferences->Spelling, that dictionary is selected.  What other step am I missing?
