I can also confirm similar behavior after the 10.1.5 and SR1 updates. For me Entourage simply loses its custom dictionary. In-line spelling continues, but words I've added to my custom dictionary (Such as my last name) start getting marked as misspelled and I cannot add words to the dictionary (the Add entry in the contextual menu is grayed out). Simply restarting Entourage will clear up the access to the custom spelling dictionary. Multi-word corrections do *not* work for me (I can happily time "might of had"). I couldn't tell you which update might be at fault since they were installed less than a day apart.
