Rules only run when the message is first encountered by Entourage. In the case of Hotmail/IMAP accounts, a few of the headers are retrieved when Entourage first encounters it, and that's all it can test against. No further information is known until one manually downloads the message; by that time the rule has already run. It then needs to be run manually.
