I'm not clear on what you may have already tried. Have you rebuilt the
database? Because these problems you describe sure sound to me like database
corruption.

Remember that you can do a rebuild and, if it somehow fails or makes things
worse, the original, unrebuilt database is still available; you can revert
simply by deleting the rebuilt one and renaming the original (which had the
date appended to its name by the rebuild process).
