Entourage expects the folder to be in your Documents folder. For OS X users,
that's the Documents folder in your User's folder. If it doesn't find an
identity, it will create a new one. That's why you are seeing a blank
identity.

If you placed your Main Identity in another location you will need to move
it to the Documents folder: Microsoft User Data folder.
