The only way I found to fix this problem was to delete the mail database,
and then create a new one with Entourage. I was able to archive all the
mailboxes that I wanted prior to deleting the old database so that I could
recover them into the new database. I used Paul Berkowitz's Sync Address
Book AppleScript to save and recovery all my addresses.

The process was a little time consuming, but not terribly difficult. In the
end, my IMAP mailboxes work like I expect.

Thanks, Barry, for your suggestion about rules being the source of the
problem.
