That is the part I am not understanding.  If I put "INBOX" in the root
folder or leave it blank, the folders appear under the INBOX folder.  But,
if I put "INBOX" in the root folder of MS Outlook or Apple Mail, the folders
do not appear under the INBOX folder.  And, I have tried both "INBOX" and
"INBOX.".

I would have to agree with you that Entourage supports the IMAP NAMESPACE
extension.  If I use Entourage to connect to a different IMAP server
software, and put in the root setting, it works.  There must be something
special about Entourage and the Courier-IMAP server software.
