I fixed my own problem: one of my Rules was turning all categories to None.

In an attempt to speed up Entourage 2004 (11.2.5), I spent most of the day,
running Paul Berkowitz's Export-Import scripts, removing Entourage, and then
reinstalling it from the original CD. I imported on the bare minimum things,
